Copyright Clearance Request Form
To help professors expedite the clearance of copyrights for any class notes that they may wish to produce we have created this clearance request form. Simply submit a list of copyrighted items that you would like our copyright department to obtain clearance for. Then when you're ready to have the class notes produced we won't have to take the time to clear the materials with the publisher.
