As of September 10, 2024, UA Passports is located on the 1st level of the Student Union Memorial Center in Room 142, next to the CatCard office.

We accept appointments only, which must be scheduled two weeks in advance. UA Passports offers convenient, full-service passport processing for new applications, along with in-house photos and family-friendly service. For urgent passport needs, please contact the National Passport Information Center at 1-877-487-2778 or visit the Western Passport Center website.

To avoid having to make multiple trips, please read this page carefully before you come.

New Passport Processing

Passport Books

Globally recognized and accepted for all forms of travel.

Choose from two sizes: the standard 26-page book or the larger 50-page book. The larger option is offered at no extra cost and is recommended for frequent travelers.

Passport Cards

This wallet-sized card is ideal for land and some sea travel to the U.S., Mexico, Canada, Bermuda, and the Caribbean. It can be used as federal identification if your driver’s license doesn’t meet 
new requirements.

Starting May 7, 2025, a “Real ID” will be needed for domestic flights. Arizona licenses without a gold star won't be accepted, but a passport card or book will fulfill federal requirements.

When applying at the UA Passport office, your fees must be separated into 2 payments as shown below. Governmental Application Fee and Execution Fee CAN NOT be combined or paid in cash.

*Expedited Processing: U.S. Department of State offers expedited processing for an additional fee (shown above) which guarantees the new passport will be received faster than standard processing, for current processing times visit travel.state.gov. This fee is charged per application and should be combined with your other Application Fees to the U.S. Department of State.

**1-2 Business Day Return Delivery (book only): This is the quickest way to receive your passport, once it is processed. The fee guarantees express mailing of your passport to you once it has been produced by the Department of State. This fee is charged per application and should be combined with your other Application Fees to the U.S. Department of State. 1-2 Day Business Return Delivery is not available for the Passport Card.

***File Search Fee: You may request a file search instead of submitting evidence of U.S. citizenship if:

  • You have been issued a U.S. passport or Consular Report of Birth Abroad in the past, and
  • You cannot submit it with your application

The U.S. Department of State requires the collection of an Execution Fee at all Passport Acceptance Facilities.

Required Documents

All new passport applications require the following:

  • Printed single sided.
  • Completed in BLACK ink.
  • If an error is made, please safely dispose of your application and fill out a new one. The Department of State will not accept applications with sections crossed through or correction tape/fluid.

DS-11 form

  • Original or Certified Copy of U.S. Birth Certificate, OR
  • Original Certificate of Naturalization, OR
  • Original or Certified Copy of Consular Report of Birth Abroad, OR
  • Prior U.S. Passport Book and/or Card

To apply for a minor, please closely follow the steps described on this page.

Please note that proof of parental relationship and consent is required from both parents for minors under the age of 16. Both parents must accompany the minor to their in-person appointment. If both parents are not able to attend, you have two options:

  • Bring in a completed and notarized Form DS-3053 statement of consent with a front and back copy of the absent parent’s photo identification,
  • OR have proof of sole custody in the form of an original or certified copy of a court order or death certificate.

Passport Renewal Processing

Renewing your passport? You may qualify to use a DS-82 application. 

In order to qualify for a DS-82 renewal by mail form, you must be able to answer yes to all the following questions*:

  1. Can you submit your most recent passport book and/or card?
  2. Were you at least 16 years old when your most recent passport book and/or card was issued?
  3. Was your most recent passport book and/or card issued 15 years ago or less?
  4. Was your most recent passport book and/or card a full 10-year validity passport?
  5. Is your name the same now as it was when your most recent passport book and/or card was issued?
    • If your name is NOT the same now, can you provide proper documentation of your name change via court order, marriage certificate, divorce decree, etc.?

*Even if you meet all the criteria above, you cannot submit a DS-82 renewal by mail if your most recent passport book and/or card was either LOST or DAMAGED. If your passport book and/or card was lost or damaged, you must apply in person using the DS-11 Passport Application.

If you answered yes to all the questions above, use the DS-82 form, and follow the instructions on the Travel.State.Gov website to submit your passport renewal.

Please do NOT bring in a completed DS-82 renewal form to UA Passports.

You do not qualify for a renewal. Please complete the DS-11 Passport Application instead and schedule an appointment with us. A $35 execution fee is assessed for all DS-11 Passport Applications. The processing time for your application will be the same with either application. Both the DS-82 and the DS-11 application can be expedited for faster processing.

Yes, you need an appointment. UA Passports no longer accepts walk-ins. Please schedule a passport processing appointment

Our business hours can be very busy and we are not always available to answer the phone. The best way to reach us is via email at INTL-Passport@arizona.edu. We typically answer within one business day.

You can check at travel.state.gov.
 

You need your first and last name, date of birth, and last four digits of your Social Security Number.

No, once we execute the application, it becomes the property of the Department of State. We cannot provide a tracking number.

The average time per applicant is 15-20 minutes.

Application Process

You need 6 things:

  1. A completed DS-11 form (we do not execute renewals with DS-82s).
  2. Citizenship evidence.
  3. An acceptable ID.
  4. Name change information (if applicable).
  5. Personal check, cashier's check, or money order for Department of State fees.
  6. Credit or debit card for University of Arizona fees.

Unfortunately, no. This is not allowed per the Department of State. If you are unable to fill out your form, you will need to obtain help with that prior to coming to UA Passports.

The Department of State only accepts money orders, cashier's checks, or personal checks for their passport fees.

The $35 is a UA Passports execution fee set by the Department of State.

You can obtain a cashiers check or money order from the following locations:

  • Your bank
  • Most grocery stores
  • Some convenience stores

No, it doesn’t need to be your own personal check, but you must have consent to use it.

Yes, you must fill out at least your parent's name, gender, and citizenship. 

Yes, the Department of State requires original documents for verification before issuing a passport. You will get them back after the passport book or card has been issued in a separate envelope via USPS.

This process can take 8-10 weeks after your passport has been issued.

Passport Types & Forms

The passport book is for flying internationally, while the card is used for land travel and some cruises. The card can also serve as a secondary ID. For more information please visit the U.S Department of State website.

DS-11 is for first-time applicants, renewing 5-year passports and adults who got their last passport more than 15 years ago. The DS-11 requires execution by an agent and a $35 execution fee. 

A DS-82 application is a mail-in only form for people who need to renew their 10-year passport issued less than 15 years ago. It does not need execution and does not need an execution fee.

For 5-year books, use the DS-11 application.

Expedited Services & Lost Passports

It means your passport will be processed as quickly as possible for an extra $60 fee. Check travel.state.gov for current processing times.

The only way to get your product sooner than expedited processing is to contact the National Passport Information Center at 1-877-487-2778 no sooner than two weeks before travel to request an urgent appointment. No walk-ins are allowed at the Department of State facilities. You must file a report with the Department of State. After you report your passport missing you will need to fill out a DS-11 application and include either the email confirmation from the Department of State or a DS-63 Lost or Stolen form. You will also need to resubmit citizenship evidence. 

We secure all your documents in a locked cabinet BEFORE being mailed out. Once we execute your application and it leaves the facility, it is out of our control. You must call the Department of State to ask about your missing documents. 

Minors & Special Cases

Yes, both parents must sign the application unless the child is 16-17 years old. In that case, only one parent needs to sign.

If both parents are not able to attend, you have two options:

  • Bring in a completed and notarized Form DS-3053 statement of consent with a front and back copy of the absent parent’s photo identification,
  • OR have proof of sole custody in the form of an original or certified copy of a court order or death certificate.

Photo Services & Corrections

Yes, but we don’t offer digital photos. Walgreens or CVS may provide digital services.

You will need to fill out a form DS-5504 and follow the mailing directions on that form. You do not need to fill out another application, nor do you need to come to an acceptance facility. Please see the Change or Correct a Passport section of travel.state.gov for more information.

No, it will be mailed to the address on your application.

Acceptance agents are not allowed to execute renewals. Visit travel.state.gov for more information.

Unfortunately, no. You can track your application status through travel.state.gov. If you have already applied and have urgent travel, you must call 1-877-487-2778 to get more information. 

We are an acceptance facility and are unable to print passport books or cards. 

We are not able to look up any information. You must call the number provided by the Department of State or travel.state.gov to report non-receipt.
